An exciting opportunity has arisen for motivated and enthusiastic Band 2 Health Care Support Workers to join our Continuing Health Care Service within the Swansea Locality. This is an excellent opportunity to join a vibrant team. We offer a friendly and supportive working environment with excellent development opportunities with support and supervision of registered nurse team leaders and senior HCSW staff. A full competency based, induction and training programme will be provided.

The Continuing Care Team delivers planned care to patients with complex needs, within their own homes. As a HCSW band 2 you will be working in a team with registered nurses and/or senior HCSW staff and also other health professionals to provide high standards of patient care within an agreed care plan.

The post h! older will be expected to have some experience of care provisi! on. The ability to travel between sites in a timely manner is essential.
